Instructions: When you click on the link the Registration form, your screen will display the registration form along with a new toolbar near the top of the window. The toolbar has an in the far left corner.

To fill out the form:

•	If the pointer is not in the form of a hand , go to Acrobat Toolbar and click on the hand.
•	The arrow should be in the form of a hand as you move it around in the form.
•	Move the hand until it is over the first field where you need to enter information.
•	At this point the hand will change to the pointer. Click on the field to begin entering text.
•	To move to the next field, hit the tab key.
•	Fill out all of the appropriate fields.

If you need to go back to the field for which you have already entered information, simply position the pointer over that field and click.

When you have completed the form, print out the form and mail it with your payment.

To print the form:

•	Be sure to hit the "Enter" key after you have completed all the fields, or the text will not print.
•	Go to the toolbar with the in the far left corner.
•	Click on the picture that looks like a printer.
•	Click on the okay button.
•	After the form has printed, you can return to the site by clicking on the back button of your browser.
